2005 Community Grants

Recipients are organized according to the Foundation's program areas: Physical Health - Clinic-Based Services, Services for Older Adults and Services for Other Special Populations. The other funding category indicates sponsorships and funds distributed separately from the normal grant process.

A summary report of the 2005 Foundation grants can be downloaded here. It includes a summary of the contributions made to the community by the Foundation's partner agencies, including the number and type of services provided and the number of people served.

Physical Health - Clinic-Based Services
El Buen Samaritano Clinic: Supported and augmented clinic's primary care services.

People’s Community Clinic: Supported and augmented clinic's primary care services and enhanced overall clinic operations and infrastructure; supported a weekly walk-in immunization clinic at People’s and the collaborative St. David’s Immunization Program with the University of Texas School of Nursing.

Round Rock Health Clinic: Supported and augmented Round Rock Health Clinic’s primary care services.

Samaritan Health Ministries: Supported primary care services for children and adults in Williamson County.

Children's Wellness Center in Del Valle: Supported the salary of a Community Health Nurse.

Volunteer Healthcare Clinic: Supported primary care and chronic disease management services for children and adults.

Services for Older Adults
Austin Groups for the Elderly: Supported a full-time Licensed Vocational Nurse in the Elderhaven Adult Day Care Center who provided regular and periodic health monitoring, dietary and nutritional counseling, and daily exercise routines designed for each client.

Faith in Action Caregivers: Supported delivery of services to older adults that enhanced their access to primary care, health promotion, and preventive health care.

Family Eldercare
: Subsidized the delivery of a continuum of services for low and moderate-income older adults in four programs: 1) Consultation and Service Coordination; 2) In-Home Care and Respite; 3) Guardianship and Bill Payer Services; 4) Lyons Gardens Senior Community.

Meals on Wheels and More
: Supported case management services for Meals on Wheels clients, nursing services, and professional dietary consultation.

Services for Other Special Populations
AISD Vision and Hearing Program: Funded vision screening machines and administrative support.

Austin Adaptive Sports Center: Supported monthly Super Sports Saturday programs for young people with physical disabilities, a ski trip to Colorado, X-Treme Sports Camp for physically disabled youths, and a scholarship fund for other special events.

Breast Cancer Resource Center: Supported staff salaries. Services included education, information, referral, and peer support.

TANDEM Project: Supported TANDEM as it offered access to primary health care, case management and support services, mental health services, and education and training in child development, sick baby care, nutrition, and general parenting skills to TANDEM teens and their children.

Other Funding
Aging Services Council: Supported the Council as it identified unmet needs among older adults in Travis County. The Aging Services Council administered a telephone survey of older adults in April 2005 and published a report of survey’s findings in January 2006.

Austin Community Foundation: Supported coordinated efforts of local non-profit agencies that provided services to Hurricane Katrina evacuees.

Austin Recovery
: The Foundation grant through 100 Women helped to provide addiction-related counseling and therapy to women and their families. The grant allowed Austin Recovery to hire a full-time Family Therapist, a full-time Child Therapist, and a part-time Family Therapist for the Multi-Family Group project.

Center for Child Protection
: Funds from the 2004 Diamonds and Ice Holiday Gala were unrestricted.

HCA Hope Fund
: Supported eligible HCA employees in New Orleans who were affected by Hurricane Katrina.

Indigent Care Collaboration
: Subsidized annual membership fees for Foundation partner clinics and supported cost analyses using the ICC database.

Johns Community Hospital
: Matched the donations that the John’s Community Hospital received through the Rock 'n Doc Gala held in December 2005. Funding supported primary care services for the uninsured and underinsured in Williamson County.

LBJ School of Public Affairs, University of Texas
: Supported Dr. David Warner’s Policy Research Project, a two-semester course offered by the Lyndon B. Johnson School of Public Affairs. The project examined community health initiatives that can reduce the amount of uncompensated care in hospitals and also enhance and improve health status and health outcomes in the community.

Lone Star Circle of Care
: Matched the donations that the Lone Star Circle of Care received through the Rock 'n Doc Gala held in December 2005. Funding supported primary care services for the uninsured and underinsured in Williamson County.

True Brilliance, Sidney Lanier High School
: Funded implementation of the "Keeping It Real" curriculum at Lanier High School.
